75 minute Initial Assessment - $225 IN-NETWORK with Blue Shield & Medicare. Co-pays TBD OUT OF NETWORK: TBD WHAT TO EXPECT - 75 minutes of a full-body evaluation that consists of finding out the root cause of your pain and implementing an exercise program for you to start doing at home.
Cancellation Policy
JERRY SHER PHYSICAL THERAPY HAS A 24 HOUR CANCELATIONS POLICY. IF A 24 HOUR NOTICE IS NOT GIVEN, A LATE CANCELLATION WILL REQUIRE A PAYMENT OF $75, WHICH IS NOT BILLED TO YOUR INSURANCE COMPANY. THE LATE FEE WILL BE DUE IN THE OFFICE UPON THE NEXT VISIT.
